Root Cause Analysis

 

Root Cause Analysis (RCA) is a tool that has its origins in Engineering and large industries. The aeronautical and aerospace industries were the first to use it as a strategy to deal with high-risk activities.
The term Root Cause Analysis indicates that there is a root cause, which contrasts with the fact that healthcare is complex and that there are usually many contributing factors to be taken into consideration in understanding why an event occurred. In view of this complexity, there is rarely a solution that covers the systems’ various dangers and vulnerabilities, which usually means that there needs to be more than one corrective action.

The aim of root cause analysis is to identity system dangers and vulnerabilities so that actions are taken to improve patient safety, avoiding future harm.
